2010 Registration Information

What a summer this has been . Many changes have happened since the end of last season. One of the most exciting is our continued use of the Internet to streamline your registration process. A couple of key points about this year's registration: 

Our annual player registration meeting will be taking place in August.    This meeting is intended to provide our parent members with the most current happenings. You will hear from the board about events being planned for this season such as Black Hawk games, fundraising needs, and advertising/peeler options.  This is an opportunity for you to meet your board members, catch up on activities from over the summer, be an informed member, and ask your questions.

To Register for the 2010-2011 WYHA Season

1.  Players must register with USA Hockey and provide registration information on the online registration tool.

2.  Review 2010-2011 WYHA Contract below.

3.  Register player on-line using the on-line registration link here.


4. Print and complete the Consent To Treat form for your team manager once teams have been selected.

 Bantam
(1996-1997)  $895

 Pee Wee (1998-1999)  $845

 Squirt (2000-2001)  $805

 Mite  (2002-2003) $715

Collection nights for dues:

Thursday, September 23rd   6:00pm to 8:00pm
Wednesday, October 13th   6:00pm to 8:00pm
Thursday, October 28th       6:00pm to 8:00pm

WYHA – WATERLOO JUNIOR HAWKS/WARRIOR

2010-2011 CONTRACT 

 

Please accept my son/daughter into the appropriate age group of the Waterloo Youth Hockey Association program.  We accept the following responsibilities:

 

  1. Register for annual $38.00 USA Hockey membership before taking the ice.  (Participants born in or after 2003 must register but pay no registration fee.)
  2. Pay the annual membership fees.  (See fee schedule).  Full payment is to be made by October 28, 2010, in check or money order.  NO CASH will be accepted.  100% of Advertising sales and 100% of Peeler sales will be applied to dues.  Ads must be turned in by October 28th, 2010.  In lieu of full payment of dues on October 28th, members can pay in three equal installments.  However, if utilizing this payment option, postdated checks for all installments must be given to the Treasurer by October 28, 2010.  No skater will be allowed to practice or play until dues are paid or postdated checks are given to Treasurer.
  3. Agree to abide by “Policies, Rules & Conduct” (provided below).
  4. Participate as a volunteer in committees, tournaments, league games, and other areas as requested.  This includes donating raffle items, volunteering as penalty box attendants, score keepers, etc.   ALL families must participate.
  5. Support all fundraisers approved by the Board of Directors.

 We understand that these activities are absolutely fundamental to the success of the Waterloo Youth Hockey Association and/or MWHSL and that if we do not meet these minimum requirements, we will not be allowed to continue in the WYHA program.  Membership fees paid will not be refunded and will be used to promote hockey in the Cedar Valley.

 
 

POLICIES, RULES & CONDUCT

 

By registering my son or daughter in the WYHA program, I acknowledge receipt of the POLICIES, RULES & CONDUCT of the Waterloo Junior Hawks Organization.  I/we understand that it is my/our responsibility as the parent(s) or guardian(s) of a Waterloo Junior Hawk/Waterloo Warrior to read these policies and to discuss them with our son/daughter who is a member of the Waterloo Junior Hawk Organization.  I/we understand that these policies went into effect on October 16, 1995 following approval by the WYHA Board of Directors, replacing all previous Codes of Conduct. 

Medical Release and Waiver of Liability

As the Parent/Guardian of the player now being registered (a minor), I appoint the acting representative of Waterloo Youth Hockey Association (WYHA) my agent for the purpose of authorizing emergency medical treatment of this player any time he/she is injured, and when I am not available. I indemnify and hold my said agent harmless from all costs and/or liability arising from such treatment. I hereby expressly agree to waive all claims against, and hold exempt from liability, the Association, it's officers, directors, coaches and any other person affiliated with the WYHA for any injury or injuries sustained by my child, from whatever cause, while attending, participating in, or traveling to and from organized WYHA activities. I agree to abide by the rules and policies of the WYHA and USA Hockey, and to accept the decision of their respective disciplinary authorities.

 Parent's Agreement

As the Parent/Guardian of the player being registered, I will:

  • BE POSITIVE to all players, coaches, officials, and parents.
  • Set a GOOD EXAMPLE and NOT CRITICIZE my player.
  • AVOID ALL VERBAL ABUSE.
  • Welcome visiting teams and treat them as I would like to be treated.
  • APPLAUD the good plays of both teams and encourage GOOD SPORTSMANSHIP.
  • ENJOY the game. I will attempt to learn about the ice hockey and VOLUNTEER to help.
  • Always INSIST that my child PLAY BY THE RULES. Children learn best by example.
  • RESPECT the USA Hockey Zero Tolerance Policy towards unsportsmanlike behavior.
  • Not be critical of the hockey program, officials, or other players in front of children. I understand that I can speak directly to the Manager, the Coach or a member of the Board.
  • BE POSITIVE. I will applaud a good effort in both victory and defeat.
  • Recognize the importance of the volunteer coaches and support them. They are important to the development of my child and the sport.
  • Be interested in all the players, ask questions, and congratulate them on their effort and achievements.
  • HONOR the WYHA Parent's Code of Conduct.
  • HAVE FUN and remember that ice hockey is a game. Kids play hockey for fun!

Abide by the Rules

As parent/guardian my player and I hereby agree to abide by all of the published rules and non-published rules and regulations of USA Hockey and Waterloo Youth Hockey Association.
